Medical Billing Services in Colorado
Go Medical Billing serves CO practices from Denver to Colorado Springs and across the state. Our team knows your local payers, Health First Colorado (the state's Medicaid program brand), and Colorado specific billing rules.
Medical Billing for Colorado Physicians
Colorado has about 16,000 physicians and an unusual Medicaid program structure. Health First Colorado does not use traditional MCOs for most members. Instead it uses Regional Accountable Entities (RAEs), which are regional organizations responsible for coordinating physical and behavioral health care, paying behavioral health providers, and managing care for Medicaid members in their region. The Accountable Care Collaborative (ACC) program is now in Phase III as of July 2025. There are five RAEs covering different regions of the state. The commercial market is dominated by Anthem Blue Cross Blue Shield of Colorado, with strong competition from Kaiser Permanente Colorado (especially along the Front Range) and UnitedHealthcare. Denver is the largest metro and is anchored by UCHealth (University of Colorado Health), HealthONE (HCA subsidiary), and SCL Health (now part of Intermountain Health after the 2022 merger). Colorado adopted Medicaid expansion in 2014. The state has high physician concentration along the Front Range (Denver, Boulder, Fort Collins, Colorado Springs) and much thinner coverage in the western mountain communities and the Eastern Plains.
Go Medical Billing is headquartered in Miramar, Florida, and serves physician practices in all 50 states. Whether you're a solo provider in Lakewood or a multi-specialty group in Denver, our team handles your billing with CO specific payer knowledge and AAPC-certified coding accuracy.
Colorado Billing Challenges
Every state has its own billing complexities. Here are the CO specific challenges our team navigates daily.
Colorado's Medicaid program uses Regional Accountable Entities (RAEs) instead of traditional MCOs for most members. RAEs coordinate care and pay behavioral health providers but most physical health claims still go fee-for-service to the state. This dual structure confuses billing teams used to MCO-only models in other states.
ACC Phase III launched July 2025 with new RAE contracts and updated behavioral health policies. Practices that bill behavioral health services have to follow the new Phase III rules including changed prior auth and credentialing requirements.
Kaiser Permanente Colorado operates as an integrated payer-provider along the Front Range. Out-of-network billing into Kaiser is much harder than into commercial PPO plans, and Kaiser patients seen out-of-network require pre-authorization that other carriers do not require.
Colorado's commercial market includes several health-tech and direct-primary-care players that do not operate like traditional insurance. Practices billing these alternative payers need to know which arrangements apply (capitated, fee-for-service, subscription, etc.).
Colorado's Medicare MAC is Novitas Solutions (Jurisdiction H), shared with AR, LA, MS, NM, OK, and TX. LCDs are written for the whole jurisdiction, which sometimes creates conflicts with Colorado-specific coverage expectations for specialties like behavioral health and ski-related orthopedics.

Colorado Billing Facts You Should Know
Colorado uses Regional Accountable Entities (RAEs) instead of traditional MCOs for most Medicaid members. The Accountable Care Collaborative (ACC) program coordinates physical and behavioral health through RAEs rather than fully capitated managed care plans.
ACC Phase III launched July 2025 with new RAE contracts, updated behavioral health benefits, and changed credentialing requirements.
Kaiser Permanente Colorado is one of Kaiser's largest regions outside California and operates an integrated payer-provider model along the Front Range. It competes with Anthem BCBS for commercial market share.
SCL Health merged with Intermountain Healthcare in 2022 to form Intermountain Health. Several Colorado hospitals rebranded as a result, which required practices to update their provider directories and referral workflows.
Colorado Payer Environment
We know every major payer operating in Colorado and their specific billing rules.
Anthem Blue Cross Blue Shield of Colorado
Anthem Blue Cross Blue Shield of Colorado holds the largest share of Colorado's commercial insurance market. We process Anthem Blue Cross Blue Shield of Colorado claims daily and know their CO specific fee schedules, prior authorization portal, provider enrollment process, and appeal timelines. We also work with Kaiser Permanente Colorado, UnitedHealthcare, Cigna, Bright HealthCare, Friday Health Plans, and every other commercial payer operating in Colorado.
Health First Colorado (the state's Medicaid program brand)
Health First Colorado (the state's Medicaid program brand) routes beneficiaries through managed care organizations: Colorado Access (Regional Accountable Entity), Rocky Mountain Health Plans (RMHP, a UnitedHealthcare subsidiary), Northeast Health Partners (RAE), Health Colorado Inc (RAE), Community Care Health Plan of Colorado (RAE), Carelon (formerly Beacon, RAE behavioral health). Each MCO has its own enrollment process, authorization requirements, fee schedules, and claims submission rules. We credential and bill with every Health First Colorado (the state's Medicaid program brand) plan, handling the variation so your staff doesn't have to track each MCO's requirements.
Medicare (Novitas Solutions (Jurisdiction H))
Medicare Part B claims in Colorado are processed by Novitas Solutions (Jurisdiction H). Each MAC has its own Local Coverage Determinations (LCDs) that affect what's covered. We apply Novitas Solutions (Jurisdiction H) specific policies to every CO Medicare claim to prevent medical necessity denials.
Colorado Regulations
Colorado prompt-pay law (CRS 10-16-106.5) requires commercial insurers to pay clean claims within 30 days of electronic submission and 45 days for paper. Late payments accrue 10 percent annual interest after the deadline.. Our team ensures your billing practices comply with both Colorado state law and federal No Surprises Act requirements.
